What is UCL Repair?

Ulnar Collateral Ligament (UCL) Repair is a surgical procedure used to restore stability and strength to the inner (medial) side of the elbow. The UCL is a key stabilizing ligament, especially in athletes who perform repetitive overhead or throwing motions. A UCL repair involves reattaching the torn ligament directly to the bone, often reinforced with a strong internal brace. This procedure is typically performed when the ligament tear is partial or located at the bone attachment site, allowing it to heal effectively without full reconstruction.

When is UCL Repair Recommended?

UCL repair is typically recommended for patients who:

  • Have a partial UCL tear or avulsion (ligament pulled off the bone).
  • Experience pain, weakness, or instability during throwing or overhead activities.
  • Have failed non-surgical treatments, including rest, physical therapy, and injections.
  • Are younger athletes or patients with healthy tissue and minimal ligament degeneration.
  • Require a faster return to sports or high-demand activities compared to reconstruction.

How is UCL Repair Performed?

The procedure is performed under regional or general anesthesia. The surgeon makes a small incision along the inner elbow, identifies the torn ligament, and reattaches it to the bone using sutures and anchors. In many cases, a collagen-coated internal brace is added for additional strength and protection during healing. Unlike traditional UCL reconstruction (Tommy John surgery), repair preserves the native ligament, resulting in a shorter recovery period.

What to Expect during Recovery after UCL Repair?

Recovery involves initial immobilization in a brace or splint, followed by gradual physical therapy. Early motion exercises begin within a few weeks, with progressive strengthening and throwing programs introduced later. Most athletes can return to competitive sports within 4 to 6 months, compared to 9 to 12 months for reconstruction.
