We are happy to hear from you. Please contact us using the information below:

Please arrive 10-15 minutes early for your appointment for registration and to fill in a health questionnaire.

When you come for your appointment, please bring the following:

• Medicare card & Referral letter
• Have your insurance information
• If possible, copies of any operation records, medical records, x-rays, MRIs, CT scans and so on from prior doctor visits (optional)
• If you have had Weight Loss Surgery elsewhere, please bring a copy of your operation report

If you need to cancel an appointment

Please telephone the office during business hours and provide at least 1 day’s notice if you need to cancel or reschedule your appointment. This allows us to offer your appointment time to patients on our waiting list.

We recognize that your time is valuable, and we make every effort to stay on schedule. Occasionally, emergencies or patients requiring additional time may cause delays beyond our control. We sincerely apologize if you experience any waiting time.
