The Osteochondral Allograft Transplant System (OATS) is a surgical procedure designed to repair damaged cartilage and underlying bone in the knee joint. It involves transplanting healthy cartilage and bone (known as an osteochondral graft) from a donor into the area of cartilage loss. This restores the joint surface, reduces pain, and helps patients return to active lifestyles. OATS is particularly useful for focal cartilage defects that do not heal on their own and can delay or prevent the need for joint replacement.

When is OATS Recommended?

OATS is typically recommended for patients who:

  • Have localized cartilage damage due to injury or early osteoarthritis
  • Experience chronic knee pain, swelling, or catching that limits activity
  • Are younger or active adults who wish to avoid joint replacement
  • Have failed prior cartilage repair procedures, such as microfracture
  • Have osteochondral defects larger than 1–2 cm that require bone support

How is OATS Performed?

The surgery is performed under regional or general anesthesia, often arthroscopically or through a small open incision. The surgeon removes the damaged cartilage and bone plug from the affected area and replaces it with a matching plug of healthy cartilage and bone from a donor (allograft). The graft is precisely shaped and press-fit into place, restoring a smooth, stable joint surface.

What to Expect during Recovery after OATS?

After surgery, patients typically use crutches for several weeks to protect the graft. Physical therapy begins early to restore motion while avoiding excessive stress on the knee. Gradual strengthening follows, with return to full activities or sports usually taking 6–9 months, depending on healing and graft integration.
