What is ORIF of the Proximal Tibia?
Open Reduction and Internal Fixation (ORIF) of the proximal tibia is a surgical procedure used to treat fractures involving the upper part of the shinbone, including the tibial spine (where the ACL attaches) and the tibial plateau (the top surface of the tibia that forms part of the knee joint). These fractures can disrupt knee alignment and stability, often resulting from high-energy trauma, sports injuries, or motor vehicle accidents. ORIF restores the bone’s normal anatomy and stabilizes it with plates, screws, or wires to promote proper healing and joint function.
When is ORIF Proximal Tibia Recommended?
ORIF is typically recommended when:
- The fracture is displaced or involves joint surface depression
- There is instability or ligament involvement (especially ACL avulsion)
- The fracture is open or associated with soft tissue injury
- Non-surgical methods fail to maintain proper alignment
- The goal is to restore joint congruence and prevent post-traumatic arthritis
How is ORIF Proximal Tibia Performed?
The procedure is performed under general or regional anesthesia. The surgeon makes an incision over the knee or upper shin, carefully repositions the bone fragments (open reduction), and secures them with plates, screws, or pins (internal fixation). In tibial plateau fractures, bone grafts or substitutes may be used to fill gaps and restore joint height. Once fixation is stable, the incision is closed, and the leg is immobilized to protect the repair.
What to Expect during Recovery after ORIF Proximal Tibia?
Patients are typically hospitalized for a few days. Early range-of-motion exercises are encouraged to prevent stiffness, while weight-bearing is delayed until the bone shows signs of healing. Physical therapy focuses on improving flexibility, strength, and balance. Most patients recover in 4 to 6 months, though complex fractures may take longer.
