What is ORIF of the Patella?
Open Reduction and Internal Fixation (ORIF) of the patella is a surgical procedure performed to repair a fractured kneecap. The patella plays an important role in knee extension by connecting the quadriceps muscles to the shinbone. When it breaks due to trauma or a direct fall, it can severely impair knee movement. ORIF restores the bone’s normal position (open reduction) and stabilizes it using metal screws, wires, or plates (internal fixation) to ensure proper healing and knee function.
When is ORIF Patella Recommended?
ORIF of the patella is recommended when:
- The fracture is displaced, causing separation of bone fragments
- The patient is unable to straighten or lift the leg
- There is an open fracture (bone visible through the skin)
- Non-surgical treatments fail to maintain alignment
- There is significant joint surface involvement that may affect mobility
How is ORIF Patella Performed?
The procedure is done under regional or general anesthesia. The surgeon makes an incision over the kneecap, repositions the fractured bone fragments into their normal alignment, and stabilizes them with metal wires (tension band wiring), screws, or small plates. These devices hold the patella in place while it heals. The wound is then closed, and the knee is immobilized in a brace or splint to protect the repair.
What to Expect during Recovery after ORIF Patella?
After surgery, the knee is typically immobilized for a short period before starting physical therapy. Early exercises help restore motion and prevent stiffness. Weight-bearing is gradually increased under medical supervision. Full recovery and return to normal activity usually occur within 3 to 6 months, depending on the severity of the fracture and healing progress.
What are the Risks of ORIF Patella?
Potential risks include:
- Infection or wound complications
- Hardware irritation or need for removal
- Stiffness or reduced knee motion
- Nonunion or delayed bone healing
- Blood clots (deep vein thrombosis)
- Persistent pain or weakness
What are the Benefits of ORIF Patella?
The major benefits include:
- Restoration of knee stability and strength
- Accurate alignment of the kneecap
- Improved ability to extend and straighten the leg
- Faster rehabilitation and return to activity
- Reduced risk of long-term joint stiffness or arthritis
