What is ORIF of the Olecranon?

Open Reduction and Internal Fixation (ORIF) of the olecranon is a surgical procedure used to repair fractures of the olecranon, the bony tip of the elbow that forms part of the ulna. The olecranon is critical for elbow extension and stability. When fractured, it can cause significant pain, loss of motion, and difficulty straightening the arm. ORIF restores the normal anatomy of the elbow by realigning the bone fragments (open reduction) and securing them with metal plates, screws, or wires (internal fixation) to promote stable healing.

When is ORIF Olecranon Recommended?

This procedure is recommended for patients who:

  • Have displaced or comminuted (multi-fragmented) fractures of the olecranon
  • Experience inability to extend the elbow due to triceps tendon disruption
  • Have joint instability or misalignment after injury
  • Are active individuals requiring full elbow motion and strength
  • Have failed non-surgical management or persistent pain after immobilization

How is ORIF Olecranon Performed?

The procedure is performed under regional or general anesthesia. An incision is made along the back of the elbow to expose the fracture. The surgeon repositions the bone fragments to restore the normal contour of the ulna and secures them using tension band wiring, plates, or screws, depending on the fracture type. Once the bone is stabilized, the incision is closed, and the arm is placed in a splint or brace to protect the repair during early healing.

What to Expect during Recovery after ORIF Olecranon?

Following surgery, patients typically wear a splint for a short period to protect the elbow. Early range-of-motion exercises are started within a few weeks to prevent stiffness while maintaining protection for the repair. Light use of the arm usually resumes after 6–8 weeks, with full strength and motion returning over 3–6 months, depending on healing and adherence to physical therapy.

  • Infection or wound healing problems
