What Is ORIF for a Distal Fibula/Ankle Fracture?

Open Reduction and Internal Fixation (ORIF) is a surgical procedure used to repair a broken distal fibula, the outer bone of the ankle. When an ankle fracture is unstable or displaced, ORIF realigns the bone fragments (open reduction) and secures them using metal plates, screws, or other fixation devices (internal fixation). This restores proper ankle alignment, stability, and function to allow proper healing.

When Is ORIF for a Distal Fibula/Ankle Fracture Recommended?

ORIF is typically recommended when:

  • The fracture is displaced or the bones are misaligned
  • The ankle joint is unstable
  • The fracture involves syndesmotic ligament injury
  • The fracture cannot be treated effectively with casting alone
  • There is an open fracture needing immediate stabilization
  • The patient is active and requires stable ankle function for mobility or sports

How Is ORIF for a Distal Fibula/Ankle Fracture Performed?

During ORIF, the surgeon makes an incision along the outer ankle to reach the fracture site. The bone fragments are carefully repositioned into their natural alignment, and metal plates and screws are secured to hold the bones together. The incision is then closed, and the ankle is protected in a splint or boot. The procedure is performed under regional or general anesthesia and usually takes one to two hours.

What to Expect During Recovery After ORIF for a Distal Fibula/Ankle Fracture?

Recovery typically includes a period of non-weight-bearing using crutches. Swelling and discomfort gradually improve with elevation, icing, and prescribed medication. Physical therapy begins once the bone shows signs of healing to help restore range of motion, strength, balance, and gait. Most patients regain normal function within three to six months.

What Are the Risks of ORIF for a Distal Fibula/Ankle Fracture?

Common risks include:

  • Infection
  • Blood clots
  • Hardware irritation or failure
  • Nerve or blood vessel injury
  • Delayed union or nonunion

What Are the Benefits of ORIF for a Distal Fibula/Ankle Fracture?

Key benefits include:

  • Restored ankle alignment and stability
  • Reliable bone healing
  • Faster functional recovery
  • Lower risk of long-term joint problems
  • Improved mobility and strength
