What is Meniscal Repair?

The meniscus is a C-shaped piece of cartilage in the knee that acts as a cushion between the thighbone (femur) and shinbone (tibia). When torn, it can cause pain, swelling, and knee instability. Meniscal repair is a surgical procedure that preserves and repairs the damaged cartilage, rather than removing it. Depending on the location and type of tear, the surgeon may choose one of three techniques: All-Inside, Inside-Out, or Outside-In. These approaches differ in how the surgeon accesses and secures the tear, but all aim to promote natural healing and maintain knee function.

When is Meniscal Repair Recommended?

Meniscal repair is typically recommended when:

  • The tear is located in the outer (vascular) zone of the meniscus, where blood supply is adequate for healing.
  • The tear is large, complex, or causing mechanical symptoms like locking or catching.
  • The patient is young or physically active and requires joint preservation.
  • Associated procedures, such as ACL reconstruction, are being performed.
  • The meniscus is repairable rather than irreversibly damaged.

How is Meniscal Repair Performed?

The procedure is done arthroscopically through small incisions.

  • All-Inside Repair: Specialized devices are used entirely within the joint, minimizing external incisions.
  • Inside-Out Repair: Sutures are passed from inside the knee to the outside, where they are tied over the joint capsule.
  • Outside-In Repair: Sutures are passed from outside the knee into the joint and tied externally.

The choice of method depends on tear location, pattern, and surgeon preference.

What to Expect During Recovery After Meniscal Repair?

Recovery includes limited weight-bearing for several weeks and gradual increase in motion. Physical therapy helps restore strength and mobility. Full return to sports may take 4–6 months, depending on healing progress.
