What is LUCL Repair?

Lateral Ulnar Collateral Ligament (LUCL) Repair is a surgical procedure that restores stability to the outer (lateral) side of the elbow. The LUCL is a key component of the elbow’s lateral ligament complex and helps prevent the joint from rotating or dislocating abnormally, a condition known as posterolateral rotatory instability (PLRI). LUCL repair involves reattaching the torn ligament to the bone, restoring the elbow’s normal alignment and function.

When is LUCL Repair Recommended?

LUCL repair is typically recommended for patients who:

  • Have acute LUCL tears resulting from elbow dislocation or trauma.
  • Experience elbow instability, especially when rotating the forearm outward.
  • Have pain, clicking, or a sense of looseness on the outer side of the elbow.
  • Have failed non-surgical treatments such as bracing or physical therapy.
  • Are younger patients or athletes with healthy ligament tissue suitable for repair.

How is LUCL Repair Performed?

The surgery is performed under regional or general anesthesia. The surgeon makes a small incision along the outer aspect of the elbow, identifies the torn LUCL, and reattaches it to the lateral epicondyle (the bony prominence of the humerus) using sutures or small anchors. In some cases, additional stabilization may be provided with a suture or internal brace augmentation to support healing and prevent re-injury.

What to Expect during Recovery after LUCL Repair?

After surgery, the elbow is placed in a brace to protect the repair. Early rehabilitation focuses on controlled motion to prevent stiffness while maintaining stability. Gradual strengthening begins after several weeks, with full recovery and return to activities typically achieved in 4 to 6 months, depending on the severity of the injury and patient compliance with therapy.
