What is Biologic Tuberoplasty?

Biologic tuberoplasty is an innovative, joint-preserving procedure designed to treat irreparable rotator cuff tears in the shoulder. Instead of replacing the joint or attempting complex tendon reconstruction, this technique involves placing a biologic graft which is often made from acellular dermal tissue, over the greater tuberosity of the humerus. The graft acts as a smooth cushion between the humeral head and acromion, reducing bone-on-bone contact, alleviating pain, and restoring shoulder mechanics.

When is Biologic Tuberoplasty Recommended?

This procedure is typically recommended for patients who:

  • Have irreparable rotator cuff tears that cannot be surgically repaired.
  • Experience painful shoulder function despite non-surgical treatments.
  • Have minimal arthritis in the glenohumeral joint.
  • Wish to avoid shoulder replacement surgery or are not ideal candidates for it.
  • Are active older adults seeking to maintain mobility and strength without a prosthetic joint.

How is Biologic Tuberoplasty Performed?

The procedure is usually performed arthroscopically. The surgeon cleans and prepares the greater tuberosity, then secures the biologic graft over it using sutures or anchors. The graft serves as a protective barrier, restoring the natural gliding surface between the humeral head and acromion. This helps maintain joint spacing and improves comfort during shoulder movement.

What to Expect during Recovery after Biologic Tuberoplasty?

Recovery typically involves wearing a sling for several weeks to protect the shoulder, followed by guided physical therapy to restore range of motion and strength. Most patients experience gradual pain relief and improved shoulder function over 3 to 6 months.

What are the Risks of Biologic Tuberoplasty?

Like any surgery, potential risks include:

  • Infection or inflammation
  • Graft failure or displacement
  • Shoulder stiffness
  • Incomplete pain relief

What are the Benefits of Biologic Tuberoplasty?

Key benefits of this procedure include:

  • Significant pain reduction
  • Preservation of the native joint
  • Minimally invasive approach
  • Faster recovery compared to joint replacement
  • Improved shoulder stability and function
